
Work Group to Consider Universal Transfer Forms

VHCA, along with the Virginia Hospital and Healthcare Association (VHHA) and the Virginia Association of Nonprofit Homes and Services for the Aging are considering a project to develop user-friendly “universal transfer forms” to facilitate information-sharing when individuals are moving between hospitals and long term care. We are seeking input from long term care providers as part of the information gathering phase as we get started. VHHA is seeking input from acute care providers as well. As a first step, we would like to know if any of our members have already developed forms that work well for them and that could be replicated in other facilities. If you have such forms and would like to share them as a starting point in this project, please forward them by mail to Beverley Soble in the VHCA office or e-mail them to her at beverley.soble@vhca.org. Secondly, if you have specific issues or items that you believe would be beneficial to include in a universal transfer form, please submit them. We are tentatively setting a first meeting date in late August, so if you have an interest in participating in the work group contact Beverley Soble by e-mail or phone at 804.212.1697.
