
Submitting Facility Reported Incidents Forms

Federal regulations addressing long term care require nursing facilities to report certain types of alleged violations (i.e. mistreatment, neglect, abuse, injuries of unknown source, and misappropriation of resident property) and results of investigations to the Office of Licensure and Certification (OLC). The Virginia Department of Health (VDH) asks that providers fax their reports to the complaint unit at 804.527.4503 and keep a copy of the transmittal sheet to verify that it was successfully transmitted. Using the complaint unit fax number ensures the appropriate OLC staff person receives the information in a timely manner. There is no need to send a follow-up hard copy or fax to any other fax numbers. Facilities can obtain the facility reporting requirements on the Virginia Department of Health’s website.
