
Transfer Form Pilot Launched

Last July the Virginia Health Care Association and the Virginia Hospital & Healthcare Association initiated a project to develop a user-friendly “universal transfer form” to facilitate information-sharing when individuals are moving between nursing facilities and hospital emergency departments. A statewide stakeholder group consisting of nursing home, hospital, home health, hospice, Department of Health, and Department of Social Services representatives along with attorneys met on a routine basis to create a one-page form to send with residents to emergency departments. The reverse side of the form isfor the Emergency Department to complete to return with residents to the nursing facility. Feedback has been very positive about the information that is transmitted on the form which provides pertinent information to treat the resident upon admission to the Emergency Department and upon discharge back to the nursing facility. After months of making modifications to the form and developing a set of instructions, the project was finally launched on Monday, June 16th for a one-month pilot. The form will be tested through July 15th and evaluated at that time. Participating in the pilot are nine nursing facilities and six hospitals.
