Culture Change Survey Reminder

Consistent with VHCA’s ongoing commitment to resident-focused initiatives such as Quality First and Advancing Excellence, we are seeking, through a brief survey to identify opportunities to recognize and support member organizations in pursuit of Culture Change.  The objective of the survey is to provide VHCA with a quick assessment of the efforts Virginia nursing facilities are undertaking with respect to culture change initiatives. 

The survey was designed to be completed via online web entry in about five minutes based upon the respondent's working knowledge of efforts underway within their facility or community - no digging for statistics or calculating trends.  There are a total of nineteen questions.

Please assist us with this important survey by working within your organization to ensure that one response per survey is submitted.  We need to hear from all member facilities regardless of initiatives, or lack thereof, to introduce Culture Change concepts.  We ask that you complete the survey by 5:00 PM on Monday, September 8th.  All responses will be kept strictly confidential. Only aggregate survey results will be shared.
