ASSISTED LIVING SURVEY DEADLINE EXTENDED TO JANUARY 20, 2006

The deadline for completing the 2006 Overview of Assisted Living: Facts and Trends survey has been extended to Friday, Jan. 20, 2006.  If you have not yet completed the survey, please take a few moments to add your information to the data base.  The survey is divided into two parts operations and resident characteristic information.  The operations survey  should be completed by the executive director or the administrator of the facility.  The resident characteristic information should be completed by the facilitys director of nursing or wellness, or the person in charge of resident care.
